On April 3, the Center for Medicare and Medicaid Innovation (CMMI) notified applicants whether or not they were accepted for participation in the Guiding an Improved Dementia Experience (GUIDE) Model.

As a refresher, the GUIDE Model is a new initiative by the Centers for Medicare & Medicaid Services (CMS) aimed at enhancing the quality of life for individuals living with dementia and alleviating the burden on their unpaid caregivers. Announced on July 31, 2023, this eight-year, voluntary model will begin on July 1, 2024.

The GUIDE Model focuses on comprehensive care coordination and management for dementia patients. Key components include:

  • Care Coordination and Management: Providers will offer ongoing, longitudinal care that includes comprehensive assessments, personalized care plans, and 24/7 access to a support line.
  • Caregiver Support: The model emphasizes caregiver education, training, and support services, including respite care, which helps caregivers take temporary breaks from their duties.
  • Community-Based Services: The model aims to bridge the gap between clinical care and community-based services, ensuring that patients and their caregivers have access to necessary resources such as meals, transportation, and social services.
  • Health Equity: A significant goal of the GUIDE Model is to improve access to dementia care in underserved populations, particularly among Black, Hispanic, Asian Americans, Native Hawaiian, and Pacific Islander communities, who often face higher rates of dementia and greater caregiving demands.

The model will provide a monthly per-beneficiary payment to participating providers to cover the costs of these services, along with a separate payment for respite care and, in some cases, an infrastructure payment to support program development activities.
